Clinical Guidance

Key Principles for Type of HRT Selection (BMS / BGCS 2024 Consensus)

1. Following Hysterectomy

Following hysterectomy: Estrogen-only HRT. Exception: personal history of endometriosis requires continuous combined HRT to prevent reactivation of residual endometriotic implants.

2. Uterus Still in Situ

Uterus still in situ (including after radiotherapy for cervical cancer or endometrial ablation): Strictly continuous combined HRT. Progestogen dose must be proportionate to estrogen dose for endometrial protection.

3. Administration Route

Transdermal estradiol is the preferred choice for women with metabolic syndrome, obesity, risk factors for venous thromboembolism (VTE) or stroke (Grade A).

4. Non-Hormonal Therapy

Evidence-based non-hormonal approaches for vasomotor symptoms include prescription pharmacological classes — serotonin-norepinephrine reuptake inhibitors (SNRIs/SSRIs), gabapentinoids, neurokinin-3 receptor antagonists (NK3R), alongside non-pharmacological modalities (cognitive behavioral therapy, lifestyle modifications). Specific agent selection, dose titration, and treatment duration must be determined individually by the treating clinician in full accordance with authorized prescribing information and the patient comorbidity profile.

⚠️ Compounded "bioidentical" hormone therapy from specialist pharmacies is NOT recommended due to lack of regulatory oversight on purity, safety, and efficacy (Grade D).
